Understanding Platelet Rich Plasma (PRP)

Platelet-rich plasma is derived from a patient’s own blood.   It is prepared by centrifuging a patient’s blood sample yielding a higher concentration of platelet than is normally found in the blood.   The centrifuge spinning causes the blood to separate into layers that include: 1) red blood cells that are forced to the bottom of the vial, 2) White blood cells and platelets form a thin middle layer, called a buffy coat  and 3) Platelet-poor  plasma, or plasma with a low concentration of platelets that makes up the  top layer.  It is the middle layer that is extracted from the centrifuged blood that is used for the PRP injection preparation.

Plasma is the watery part of the blood that contains proteins, nutrients, glucose and other substances produced by the body.  It carries red and white blood cells, platelets and other substances that must travel in the blood.  Platelets secrete substances called growth factors that control cell division, stimulate tissue regeneration, and promote healing. The Priapus Shot ® procedure protocol involves patient selection, patient evaluation & education (including an explanation of consent), preparation of the PRP, local anesthesia, PRP injection and post-injection use of a pump on a daily basis. Other post injection steps can include: stopping smoking, CoQ10 (12), vitamin E (13), Trimix, and aerobic exercise. Protocol steps vary depending on the patient and those variations also comprise the Priapus Shot® protocol.*

The patient selection includes identifying those who may need hormonal treatment, or family counseling, or vascular surgery, as well as those who may have co-morbidities or who may be taking drugs that interfere with their health. An early report that PRP may be useful in appeared in a paper published in Urology in 2003 mentioning that, in animal models, using blood-derived growth factors injected successfully treated erectile dysfunction and also mentioned that such a strategy may be feasible in men— actually providing a way to correct the underlying pathology (1). In contrast, to ED drugs that do not correct the underlying pathology of decreased penile circulation.*

Another animal study in 2010 showed that transferring adipocyte-derived stem cells (ADSCs)   caused endothelial cell growth (new blood flow) as well as increased nitric oxide activity in the dorsal nerve.
